Transition Coordinator (Rochester, NY)

Job Overview:

The  Transition Coordinator is a collaborative position dedicated to supporting the seamless transition of disabled students into adulthood and in their journey toward independence and self-fulfillment. This role requires a strong ability to build authentic, engaging relationships with disabled youth and motivate them to explore their futures with confidence and excitement. This role works closely with youth with disabilities, schools, families, community partners, and internal staff to create accessible, inclusive experiences that help participants prepare for a successful future where they feel confident in employment settings. As the Transition Coordinator, you will be responsible for providing and enhancing career exploration, disability-related programming, and supporting high school transition plans, with a focus on the Pre-Employment Transition Services (Pre-ETS) program and the EmpowHer Roc/+ programs.

 

Disability EmpowHer Network empowers girls and women with disabilities through mentoring and transformational learning experiences. Disability EmpowHer Network is a remote organization; however, this position requires regular in-person meetings with participants in school and community settings in and around Monroe County and surrounding counties in New York.

Responsibilities:

  • Build strong, trusting relationships with disabled youth and actively engage them in career exploration, helping them feel empowered, excited, and optimistic about their futures.

  • Develop and facilitate engaging, disability-centered curriculum focused on employment readiness, self-advocacy, social skills, and independent living.

  • Coordinate and deliver programming across school and community settings, ensuring alignment with youth needs and educational goals.

  • Build and maintain relationships with participants, schools, families, and community organizations to recruit participants, promote programs, and ensure the success of participants as well as the overall program.

  • Serve as the primary point of contact for youth and families, providing individualized support, addressing concerns, and connecting participants with relevant resources.

  • Oversee program logistics including scheduling, accommodations, meals, transportation, and activity coordination.

  • Support participants in setting and reaching employment goals through mentorship, coaching, and resource navigation.

  • Supervise and collaborate with part-time program staff to ensure high-quality, consistent delivery of services of our transition services as well as our local programs serving disabled girls and women in and around Monroe County, NY.

  • Track participant progress and maintain accurate documentation and data for internal tracking and grant reporting.

  • Create outreach materials and present to participants, schools, families, and community groups to raise awareness and engagement.

  • Collect, analyze, and respond to participant feedback to inform continuous program improvement.

  • Ensure all programming is inclusive, accessible, and conducted in a safe and supportive environment.

  • Complete required training and submit reports and evaluations in a timely manner.

 

Perform other duties related to this job description and as assigned by the Director of Programs.

Qualifications:

  • Passion for engaging with and uplifting youth with disabilities by creating a fun and safe environment

  • Experience working with and providing constructive feedback and support to people with different disabilities and access needs.

  • Strong communication and interpersonal skills.

  • Empathy, patience, and a passion for empowering disabled individuals.

  • Knowledge of resources and services available for people with disabilities in NYS.

  • Ability to work independently and collaboratively with a diverse team and partners.

  • Strong time management, project coordination, and communication skills.

  • Experience with or willingness to learn Gmail, Google Docs, Microsoft Word, Google Calendar, Canva, and Meta.

  • Experience with disability advocacy preferred

  • Disability Pride required

  • Reliable transportation is required for this position to attend participant meetings and travel to various locations.

 

This position works 28 hours a week, with approximately a quarter of those hours falling on evenings and weekends. Benefits include 12 paid holidays, 2 weeks off each year when the office is closed (a late-summer break and the week between Christmas and New Years), and flexible scheduling.

 

Pay rate for this position:  $22 / hour -  $26 / hour

The salary for this employee will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training.
